OPAL Registration Form

OPAL (Ohio Permit & Liquor Licensing) is the Ohio Division of Liquor Control’s new online, account-based liquor licensing system. To learn more about what you are able to do in OPAL and for answers to other frequent questions about OPAL, go to www.com.ohio.gov/opal.
You MUST complete this registration form if you are an existing permit holder or pending applicant for a Class A (manufacturer), B (distributor), C (carryout), or D (on-premises consumption) permit. Completing this form allows us to validate your information so that we can link you to your existing liquor permit provided the information you provide us matches what we have in our database.

1. Permit Number (look for 7 digit base number with possible "-" suffix on your permit)


You MUST submit a form for each permit number that you need to be linked up to.

2. Permit Holder Contact Information


The information provided below in this section, including the email address, MUST be for someone who, according to our records, is a disclosed stakeholder on your permit record. Disclosed persons may include:
  • Shareholders
  • Members of an LLC
  • Officers in the business like CEO, VPs, Treasurers
  • Food and beverage mangers in a non-profit
  • Club officers in a D-4 permit
  • Trustees (if the permit is held by a trust)
 If you need help finding who is disclosed in our records, you can find this information on your last renewal application or by going to www.com.ohio.gov/liquorinfo and selecting the tab search who has disclosed an ownership interest in a particular liquor permit.

3. Secondary Contact Information (FULL ACCESS)


While this step is NOT required, you CAN add another user that will have FULL ACCESS to your OPAL account (additional authorizing documentation like a Power of Attorney document may be requested). FULL ACCESS means that they will be able to everything the permit holder could do like submitting new and transfer applications, managing renewals, cancelling permits, and filing to change stakeholders. If you do NOT want to add a secondary contact at this time, you can do it later within OPAL under your "Account Management" tab. Secondary Contacts could be:
  • Attorneys
  • Paralegals
  • Employees of the company
  • Additional Stakeholders
